Citroen Relay Window Van 2002

The water keeps disappearing from the water container but not leaking. It seems to be coming out from the top because it’s always wet on everything around the bottle. Also what is the hose coming off the screw cap from the water container and is it meant to be connected to anything? I think the water may be coming out of that too. Please help, I’m baffled and need to sort the problem! Thanks.
